When you twist an ankle or pull a muscle, what’s the first thing that comes to mind?
For years, people followed the RICE protocol: Rest, Ice, Compression, and Elevation.
But times have changed, and so has our understanding of soft tissue injuries. In recent years, a new approach has gained momentum—one that emphasizes more active and holistic care. Meet the PEACE & LOVE acronym- a modern, comprehensive method for treating soft tissue injuries that has left RICE in the past.
In this blog, we’ll explore why RICE is no longer considered the BEST approach for soft tissue injuries, dive into what PEACE & LOVE stands for, and explain why this method leads to better recovery outcomes for the future of your body & nervous system.
The Limitations of RICE
For decades, the RICE protocol was the go-to method for treating soft tissue injuries like sprains, strains, and bruises. The idea behind RICE was to reduce swelling and pain by limiting movement and applying ice. However, while it might provide some short-term relief, research has shown that the RICE method might not actually promote long-term healing. In fact, several studies suggest that icing, in particular, can delay the natural healing process by constricting blood vessels and reducing the delivery of essential nutrients and oxygen to the injured area.
Additionally, complete rest can lead to muscle atrophy and joint stiffness, especially in the early stages of healing. In short, RICE addresses symptoms rather than the root cause of the injury. As we now know, an injury requires more than just symptom management—it needs support that aids tissue repair and fosters movement for proper recovery. That’s where PEACE & LOVE comes in.
At Clash PTHS we are 100% advocates of getting our patients to the root cause of their issues and providing long term management.
What is PEACE & LOVE?
PEACE & LOVE is a newer, research-backed acronym that offers a more comprehensive approach to treating soft tissue injuries. It stands for:
- **P**rotection
- **E**levation
- **A**void anti-inflammatories
- **C**ompression
- **E**ducation
- **&**
- **L**oading
- **O**ptimism
- **V**ascularization
- **E**xercise
Let’s break this down in detail to understand why this method is more effective than the traditional RICE approach.
The PEACE Protocol (Immediate Care)
P: Protection
In the initial phase of injury, it’s crucial to protect the injured area from further harm. This doesn’t mean immobilizing it entirely but rather avoiding activities that could exacerbate the injury. Limiting movement for the first one to three days is often necessary to prevent additional damage, but complete rest is generally discouraged after this short period.
E: Elevation
Elevating the injured limb above the level of the heart can help reduce swelling by promoting the return of fluids to the circulatory system. This is one aspect of RICE that holds up in the PEACE protocol.
A: Avoid Anti-Inflammatories
This is one of the most critical differences from RICE. Research shows that inflammation is an essential part of the healing process. It’s the body’s natural response to injury, sending necessary healer cells to the site to begin repair.
So, why would we want to impede this from actually happening? Won’t that delay healing further- potentially causing pain for longer and limiting the results?
Using anti-inflammatory medications like NSAIDs (non-steroidal anti-inflammatory drugs) may inhibit this process, delaying recovery. The PEACE approach advises against these medications for the initial treatment of soft tissue injuries.
C: Compression
Similar to RICE, applying compression through bandages or wraps helps manage swelling and provides support to the injured area. However, the emphasis here is on light, functional compression rather than overdoing it, which could restrict blood flow and hinder healing.
E: Education
One of the most significant advancements in the PEACE & LOVE approach is patient education. When individuals understand the nature of their injury and what to expect during the recovery process, they are less likely to overreact or seek unnecessary treatments. Education empowers patients to be active participants in their recovery, helping them make informed decisions about managing their injury.
If you have been a patient at Clash PTHS previously or currently- you know each one of our clinicians spends ample amounts of time educating and debunking many myths and misunderstandings when it comes to pain, the human body, & brain.
At our clinic, we want to keep our patients in the driver’s seat of their health and one of the ways to do this is through constant education.
Knowledge is Power.
The LOVE Protocol (After Initial Care)
L: Load
Once the initial protective phase is over, the injured area needs to be gradually loaded with movement and weight. Loading means progressively reintroducing movement and stress to the injured tissue. This promotes tissue remodeling and helps to regain strength and mobility. Unlike the RICE protocol, which emphasized rest, the PEACE & LOVE approach advocates for early, controlled loading to speed up recovery.
This is where working with a Physical Therapist who understands pain & the brain at the same time is immensely beneficial. We refer to this specific education as “Pain Neuroscience Education” and “Graded Exposure.” (Make sure to ask your therapist if they are familiar with this prior to booking an appointment).
When you resume movement after an injury you have to do so by dipping your toes in the water BEFORE you fully dive in and commit.
O: Optimism
The psychological aspect of injury recovery cannot be overstated. Optimism and a positive mindset are essential for healing. Research has shown that individuals who maintain a positive outlook and believe in their body’s ability to recover tend to heal faster than those who feel pessimistic about their injury. Encouraging patients to stay optimistic and focus on gradual improvement can improve outcomes significantly.
V: Vascularization
Maintaining cardiovascular health is important during injury recovery. Low-impact aerobic exercise—such as walking, cycling, or swimming—can increase blood flow to the injured area, promoting healing while maintaining overall fitness. Vascularization is crucial because it helps deliver oxygen and nutrients to the injured tissues, accelerating the healing process.
Sometimes seeking out a Massage Therapist can be beneficial to help increase blood flow and lymphatic drainage.
Clash PTHS has many excellent and reputable Massage Therapists they are comfortable with referring you to for the best continuity of care.
E: Exercise
Finally, exercise is key to a successful recovery. Tailored, progressive exercise programs that focus on strength, flexibility, and proprioception (balance and coordination) will ensure that the injured area regains its full function. Importantly, exercise also helps prevent future injuries by building resilience in the affected tissues.
This is why having a plan of care tailored to you by a healthcare professional is crucial to your healing.
No two bodies are the same.

Why PEACE & LOVE is Superior to RICE
Now that we’ve broken down PEACE & LOVE, it’s clear why this method is becoming the gold standard in soft tissue injury treatment. Here’s why it’s better than the outdated RICE approach:
- **Focuses on Long-Term Healing**: While RICE mainly addresses symptoms like swelling and pain, PEACE & LOVE promotes healing by supporting tissue repair and gradual reintroduction of movement.
- **Avoids Over-Reliance on Rest and Ice**: Rest and ice might reduce pain in the short term, but they can delay the natural healing process. PEACE & LOVE encourages controlled movement early on and recognizes inflammation as a necessary part of recovery.
- **Holistic Approach**: The PEACE & LOVE method integrates physical, emotional, and educational components, offering a more well-rounded approach to injury management.
- **Active Participation in Recovery**: With an emphasis on education, optimism, and progressive exercise, this protocol encourages patients to take an active role in their recovery rather than relying solely on passive treatments like icing and rest.
- **Improves Recovery Outcomes**: By fostering movement, strength, and cardiovascular health, PEACE & LOVE leads to faster, more complete recoveries and reduces the risk of reinjury.
Conclusion
The PEACE & LOVE approach represents a paradigm shift in how we treat soft tissue injuries. While RICE served its purpose for many years, it is now clear that a more dynamic and holistic approach is necessary for optimal recovery. By incorporating protection, movement, mindset, and education, PEACE & LOVE not only promotes faster healing but also empowers individuals to take charge of their recovery process. If you find yourself with a soft tissue injury, remember that healing isn’t just about resting and icing—it’s about finding balance, staying positive, and gradually rebuilding your strength.
